and take a Print-out of the Member Registration Form from NUTP
Online. |

Fill in the form and submit it with a copy of your salary slip and identity card. |

Also you can obtain the Member Register Form from the NUTP Headquarters or any of the NUTP branches throughout the country. |

Send your completed form to the nearest NUTP Branch. |
 |
Don't send any money as we will bill you through Angkasa. |
